Survey development for patients with chronic disease on multidisciplinary care – Consumer Representatives

The project

The management of metabolic chronic diseases, such as type 2 diabetes, cardiovascular disease, obesity, and cancer, often require the coordinated care of multidisciplinary care teams (MDTs) within community-based primary care settings. MDTs mean various professionals are involved in the care (e.g. the GP, specialists, nurses, pharmacists).

Patients’ experiences and perceptions of how these professionals work together and operate as teams are not well understood. Asking about patient perceptions of team configurations, the dynamics within these teams, and how care is delivered in this setting can shed light on potential improvements in multidisciplinary care models.

Researchers at Curtin are conducting a study that aims to develop and implement a patient survey to capture the lived experiences of individuals with metabolic chronic conditions, focusing on their perceptions of MDT configurations in primary care and the impact these have on disease management.
