Following on from the important information you received regarding vaccination mandates for staff and students, you are required to provide evidence of your vaccination status (or exemption) to the University.
All students accessing a Western Australian Curtin University campus or venue or participating in an “in-person” activity organised by Curtin, must comply with the vaccination mandate.
Please complete the COVID-19 Vaccination Status Collection webform.
This link can also be accessed via OASIS and eStudent.
If you aren’t already logged in to OASIS you will be prompted to do so before you can access the form.
Ensure you have your vaccination dates and/or evidence ready to upload to the form. Your vaccination certificate must contain all COVID-19 vaccinations you have received, and for this reason screenshots of the ServiceWA app certificate will not be accepted. For more information on acceptable vaccination documentation, please refer to the Student Vaccination FAQs.
NOTE: Students must redact their IHI number from their vaccination certificate before uploading into OASIS. Please refer to these guidelines on how to do this.
Once you have submitted the form your evidence will be assessed and you will receive a confirmation of the outcome within 3-5 business days.
