Every new patient must complete a registration form. When you are booked as a new patient, you will receive a text message from Hotdoc which includes a link to an online registration form.
If you did not receive the link, please use the link below.
If you have an overseas health insurance policy, you must include details of the policy/membership number.
For all patients who do not have a Medicare card, we encourage that you obtain an Individual Healthcare Identifier (IHI) which allows you to obtain prescriptions electronically (via tex) and helps you keep your prescriptions safe and secure. This function is not available if you do not have an IHI number.
We require a minimum of 30 minutes notice to cancel your appointment. This will allow us to offer your appointment to another patient who may need to see a doctor.
Failure to cancel your appointment within the appropriate timeframe may result in a fee between $40-$60. This fee will need to be paid before you can make your next appointment.
Patients who have provided us with their mobile numbers will receive a text message from Hotdoc asking them to confirm or cancel their appointment. This is an automated message and requires only a Y or N response. Patients who book on the day of the consult may not receive a Hotdoc text message.
Please do not text back with requests or sentences as the message will not come through to our appointment book and your appointment may not be confirmed.

International Students (main OSHC policy holder)
Consult fees are sent directly to your insurance provider if you have a current policy*. Procedures and immunisations may incur a fee^.
Dependants of main OSHC policy holders
An out-of-pocket fee of $20.00 is applicable for all consults. This excludes children 12 years of age and under.
Overseas Students (Visitor’s Cover)
Students on a Visitor’s Cover will incur a fee^.

*OSHC policies from Allianz. BUPA, NIB, AHM and Medibank will be accepted for direct claiming. Please speak to the Receptionist if you have an OSHC policy with another provider.
^ Student sanctions may apply if your outstanding account is not settled within three weeks from the date of service. You will be contacted by email to alert you to your outstanding account.

We accept EFTPOS, Visa and Mastercard. We do not accept cash or cheques.
If you have Medicare and are entitled to a Medicare rebate, please present a bank card (cheque or savings card) to receive an instant rebate. If you do not provide us with the correct card for rebate purposes, you may need to claim yourself via the MyGov app/account.
We recommend that you download your health insurance provider’s mobile app as it is a more convenient and effective way to locate your membership details. We may request for your details from time to time.

The Curtin University Health Service has an automated text reminder system using the Hotdoc software.
We will not routinely call patients regarding normal test results. If your test result requires urgent attention, our Practice Nurse will contact you with the necessary details.
For non-urgent results, a text message will be sent to you asking to make an appointment with the GP. In some cases, you may be receive a reminder from state and national registers.
In order to have your medical records transferred to the Curtin University Health Service, please complete the form below and email it to HSManager@curtin.edu.au. Alternatively, you can hand the form into Reception at Building 109, Level 1 Curtin Perth.
If you would like your medical records transferred to another medical centre, you will need to advise your new GP that you would like your records transferred from Curtin University and they will provide you with an authority form to complete which they will then send to us to the complete the transfer. Your records will be sent encrypted to your new medical centre via Registered Post.
Note:
We will transfer your records once for free. If you request your records to be transferred to multiple practices, an admin fee of $30 will apply.

We are required to collect personal information from patients and other health care providers in order to main proper health care.
