Can someone else enquire about my student information or speak on my behalf if I am unable to?

No, unless you complete a Disclosure of Information/Proxy Authorisation Form and provide consent to another party to receive information and/or act on your behalf.

Once the form has been completed and signed by both the student and the nominated proxy, submit it to Curtin Connect via webform as shown below:

1. Log into OASIS.

2. Select ' Contact Us'.

3. Select 'Submit an online enquiry' from the 'Current Students' tile.

4. Scroll down and select the 'Student details, ID cards and documents' tile.

5. Select 'Official documents and letters' from the list to submit your enquiry and attach the disclosure form.

Due to privacy obligations Curtin University will not release any information pertaining to a student's details or account to anyone other than the student themselves. Refer to the form for further details regarding required identity documentation.
