What documentation do I need to provide with my Leave of Absence application as an international student?

If you are an International student visa holder applying for a Leave of Absence (LoA), your application will be assessed in accordance with government-legislated requirements. This means you will need to supply relevant documentation that supports the reason you wish to apply for a LoA. The following guidelines may assist you:
Student injury, illness or medical condition supporting documentation:
- Signed statement on relevant official letterhead or a completed Professional Practitioner Certificate from a Medical Practitioner or other approved health professional registered by the relevant National Medical Board outlining how circumstances have impacted your studies.
- Medical statements from online services will not be accepted unless face to face consultation occurs or a previous practitioner-patient relationship exists in accordance with the AMA Medical Certificate Guidelines, revised 2016.
- Medical statements from pharmacists are not acceptable.
- Signed statement on relevant official letterhead from Curtin University’s Counselling and Disability Services, valid Curtin Access Plan (CAP) or registered psychologist outlining how circumstances have impacted your studies.
Extenuating Personal Reasons: family issues (e.g., family injury, illness or bereavement) supporting documentation:
- Death Certificate or funeral notice.
- Police report or statement.
- Any form of supporting documentation accepted under the Student injury, illness or medical condition category listed above.
- Statutory declaration outlining the relationship to the impacted family member if this is not evident without clarification.
Unit Availability or requisite issues supporting documentation:
- A supporting statement from your Course Coordinator confirming that your unit availabilities are limited or that you have not met necessary prerequisite requirements and cannot undertake any units in the current study period.
All documentation must be provided in English, be verifiable by the University and be and attached to your Leave of Absence application (eForm).
